About this file

This document is the City of Long Beach Quality Assurance Program (QAP) dated April 17, 2024, prepared by the Public Works Department. The QAP establishes procedures for ensuring that materials incorporated into construction projects conform to contract specifications. The program is based on the Caltrans Local Assistance Procedures Manual (LAPM) dated January 2024 and Chapter 16 thereof, which provides guidelines for local agencies. The City of Long Beach QAP distinguishes between projects on and off the National Highway System (NHS), with applicable Federal Highway Administration (FHWA) regulations governing NHS projects. The program is updated every five years or more frequently if changes occur in testing frequencies, sample types, or test methods themselves.

The QAP establishes comprehensive requirements for materials laboratory management, acceptance testing (AT), independent assurance programs (IAP), and certification of sampling and testing personnel. The City will use a private consultant certified materials laboratory to perform acceptance testing on Federal-aid and designated projects under the responsible management of a California registered engineer. Materials laboratories must participate in correlation testing programs such as AASHTO Materials Reference Laboratory (AMRL), Cement and Concrete Reference Laboratory (CCRL), Caltrans' Reference Samples Program (RSP), or Caltrans Corroboration Program (CSP). Personnel must be certified by Caltrans District Materials Engineers, nationally recognized organizations such as the American Concrete Institute, or other state and local government-approved organizations. Testing equipment must be calibrated annually using devices traceable to the National Institute of Standards and Technology. The program includes detailed provisions for source inspection of manufactured materials, materials certification upon project completion, Buy America compliance requirements, and comprehensive project record-keeping organized according to Caltrans Construction Manual filing categories, with project files maintained for at least three years following final project voucher.

QUALITY ASSURANCE PROGRAM (QAP)

AGENCY: City of Long Beach

The purpose of this program is to provide assurance that the materials incorporated into the construction projects are in conformance with the contract specifications. This program should be updated every five years or more frequent if there are changes of the testing frequencies, sample types or to the test methods themselves. This OAP of the City of Long Beach is based on Caltrans Local Procedure Manual (LAPM) dated January of 2024.

Chapter 16 of this LAPM has the "Quality Assurance Program (OAP) Manual for Use by Local Agencies" and the City of Long Beach (CoLB) has followed the requirements of the guidelines stated in this manual. This program also meets the requirements of Code of Federal Regulations Chapter 23 CFR 637 that govern Transportation Quality Assurance Program. This OAP will make the distinction between projects on and off the National Highway System (NHS). For projects on the NHS, applicable Federal Highway Administration (FHWA) regulations will also govern.

DEFINITION OF TERMS

ACCEPTANCE TESTING /AT)

AT will be performed by a materials laboratory certified to perform the required tests. The tests results will be used to ensure that all materials incorporated into the project, including acceptance of unprocessed and processed aggregate, are following the contract specifications.

Test data and summary logs: Logs of all testing shall be maintained. If failing test results occur, sufficient additional acceptance sampling and testing shall be performed such that the failing tests numbers are linked to the retest numbers showing compliance of the results.

Testing methods will be in accordance with the California Test Methods (CTM) or a national recognized standard (i.e., AASHTO, ASTM, etc.) as specified in the contract

Page4 specifications. The requirements for specific acceptance testing will meet construction contract specifications.

Sample locations and frequencies may be in accordance with the contract specifications.

If not so specified in the contract specifications, samples shall be taken at the locations and frequencies as shown in Attachment #1 (Appendix D, "Acceptance Sampling and Testing Frequencies" of the Construction Manual). Attachment #4 show various test data summary logs that will be maintained. Summary log will have information that will allow to find information details on the testing date, location, technician, type of tests, compliance requirement results, actual results, retesting status on failing tests, and additional notes.

Mix Design Approval and Checking: Mix design submitted by the Contractor will be reviewed and verified for contract compliance. Concrete and HMA Pavement mix designs submitted for work within Caltrans' right-of-way will be forwarded to Caltrans for approval (after complete review and verification by the Agency).

For HMA pavement Job Mix Formula (JMF), forms CEM 3511 (submittal), CEM 3512 (Contractor testing results), and CEM 3513 (JMF Verification testing results) will be used.

HMA Production Startup Evaluation Form (CEM 3703) will be used to input all required data.

CERTIFICATION OF SAMPLING AND TESTING PERSONNEL

All samplers and testers, including project, laboratory, and consultant personnel, must possess a current certificate of proficiency for the tests performed. A copy of the certificate must be in the project files. It is important that samplers as well as testers are certified to ensure the integrity of the sample and that the sample was taken at the right time, from the right location, using the correct method, and is properly labeled.

The Joint Training and Certification Program (JTCP) was established by Caltrans, LPAs, and industry to make the certification process more efficient and to obtain consistent, reliable quality testing. The JTCP offers training and certification in hot mix asphalt, soils and aggregates, and Portland cement concrete.

For CTMs not covered by the JTCP, Caltrans will still provide certification. When test methods not covered by the JTCP or Caltrans are used, certifications must be obtained from other acceptable organizations such as ACI, or the agency/testing consultant may need to hire a second lab to perform IA. The process for qualifying sampling and testing personnel should be detailed in the LPA's Independent Assurance Program of the QAP.

IA sampling and testing is not to be used for determining quality and acceptability of material incorporated into the job. Such tests are used only for the purpose of determining the reliability of testing personnel.

INDEPENDENT ASSURANCE PROGRAM (IAPI

IAP shall be provided by personnel from the Agency's certified materials laboratory, or consultant's certified materials laboratory. IAP will be used to verify that sampling and testing procedures are being performed properly and that all testing equipment is in good condition and properly calibrated.

IAP personnel shall be certified in all required testing procedures, as part of IAP, and shall not be involved in any aspect of AT.

IAP shall be performed on every type of materials test required for the project. Proficiency tests shall be performed on Sieve Analysis, Sand Equivalent, and Cleanness Value tests.

All other types of IAPs shall be witness tests.

Poor correlation between acceptance tester's results and IAP's results may indicate probable deficiencies with the acceptance sampling and testing procedures. In cases of unresolved discrepancies, a complete review of AT shall be performed by IAP personnel, or an independent materials laboratory chosen by the Agency. IAP samples and tests are not to be used for determining compliance with contract requirements.

Compliance with contract requirements is determined only by assurance testing (AT).

REPORTING ACCEPTANCE TESTING RESULTS

The following are time periods for reporting material test results to the Project Engineer:

✓ When aggregate is sampled at material plants, test results for Sieve Analysis (CT202), Sand Equivalent (CT217) and Cleanness Value (CT227) should be submitted to the Project Engineer. At the job site, test results for compaction and maximum density should be submitted to the Project Engineer within 24 hours after sampling.

✓ When soils and aggregates are sampled at the job site:

1. Test results for Sieve Analysis, Sand Equivalent and Cleanness Value should be submitted to the Project Engineer within 72 hours after sampling.

2. Test results for "R" Value and asphalt concrete extraction should be submitted to the Resident Engineer within 96 hours after sampling.

When sampling products such as Portland Cement Concrete (PCC), cement-treated base (CTB), hot mix asphalt (HMA), and other such materials; the time of such sampling shall be varied with respect to the time of the day as far as possible, to avoid a predictable sampling routine. The reporting of AT results, if not performed by the Project Engineer's staff, shall be done on an expedited basis such as by fax or telephone.

Acceptance test results must be submitted to the resident engineer and filed in the proper category of the LAPM referred filing system.

City of Long Beach OAP

TESTING OF MANUFACTURED MATERIALS

To get encroachment permit from Caltrans 9for projects within Caltrans' right-of-way), the Agency (CoLB) will prepare a Source Inspection Quality Management Plan (SIQMP) per the guidelines outlined by the Caltrans "Source Inspection Guideline for Local Agencies (SIGLA) and submit to Caltrans OSM for approval.

The City of Long Beach (CoLB) wi ll submit a "Source Inspection Request" (see Attach~m~ent#2t(c:xrribit 16-W~orthe~LAPMf for inspection ana te sting of manufactured and prefabricated materials by their materials laboratory. A list of materials that can be typically accepted based on certificates of compliance during construction is found in Attachment #3 (Exhibit 16-T1 of LAPM). All certificates of compliance shall conform to the requirements of the contract specifications ; the certificate of compliance will clearly state the product, production date, manufacturer, specifications requirements met, lot number, date of the certificate, representation of quantity, and project EA number; the goal is to assure tracing of product quality and manufacturer.

Caltrans no longer provides source inspection services for projects of local agencies.

Caltrans may provide source inspection for projects on the SHS if terms are detailed in a cooperative agreement or encroachment permit.

For Federal-aid projects on the National Highway System (NHS), Caltrans will assist in certifying the materials laboratory, and the acceptance samplers and testers.

SIQMP IMPLEMENTATION:

For NHS projects, the Agency (CoLB) will submit the SIQMP to the Caltrans Oversight Structural Materials Representative (OSMR) for the project and include a copy to the Caltrans Project Manager or Oversight Engineer for the project. The following link contains the latest OSM contact information.

fhttp://www.dot.ca.gov/hq/esc/Translab/OSM/smdocuments/StructuralMaterialsRepresent atives.pdf)

For non-NHS projects the SIQMP will be submitted to CoLB Project Manager/Agency OSMR.

For NHS projects, the OSMR will review the SIQMP. To expedite the review and approval of the SIQMP, OSM will require the most current Plans and Specifications for the project be submitted along with the cooperative agreement. Once approved in writing, the SIQMP will be the guiding document for management of materials that are fabricated or produced outside of the jobsite.

Caltrans OSM will perform independent quality assurance (IQA) through periodic audits to ensure that the approved acceptance activities outlined in the SIQMP are followed.

OSM will communicate the IQA audit findings through periodic status reports submitted to the Oversight Engineer. These reports are based on an evaluation of the CoLB's adherence to the SIQMP. They will describe and track any SIQMP deviations. Upon completion of the source inspection activities and resolution of all outstanding IQA findings, OSM will provide a written letter stating that the project is following the approved SIQMP.

For Federal-aid projects on the National Highway System (NHS), Caltrans will assist in certifying the materials laboratory, and the acceptance samplers and testers. For Federal- aid projects off the NHS, Caltrans may be able to assist in certifying the materials laboratory, and the acceptance samplers and testers.

MATERIALS CERTIFICATION

Upon completion of a Federal-aid project, the registered engineer shall complete a "Materials Certificate." The Agency (CoLB) shall include a "Materials Certificate" in the Report of Expenditures submitted to the Caltrans District Director, Attention: District Local Assistance Engineer. A copy of the "Materials Certificate" shall also be included in the Agency's construction records. The Project Engineer in charge of the construction function for CoLB shall sign the certificate. All materials incorporated into the work which did not conform to specifications must be explained and justified on the "Materials Certification", including changes by virtue of contract change orders. See Attachment #5 for an example (Exhibit 17-G of the LAPM). A copy of the Materials Certificate will be submitted to Caltrans' DLAE also.

BUY AMERICA REQUIREMENTS:

The FHWA's Buy America policies, per 23CFR 635.410 and Section 70914 of the Build America, Buy America Act, require a domestic manufacturing process for all steel or iron products that are permanently incorporated in a Federal-aid highway construction project. Construction provisions incorporates the FHWA Buy America requirements; it is mandatory to comply with these requirements and have documents accordingly.

Contractors must provide a signed certification statement regarding the manufacture of the iron and steel products to meet Buy America requirements. Each certification should reference the steel heat number or other tracing methods, including statement showing the location of fabrication plant and information about the original mill work. The location of the final step where a product is coated with paint, or galvanized, shall conform to the Buy America provisions.

RECORDS

All material records of samples and tests, material releases and certificates of compliance for the construction project shall be incorporated into the Project Engineer's project file. If a Federal-aid project:

✓ The files shall be organized as described in Section 16.3 "Maintaining Project Records" of the Local Assistance Procedures Manual (LAPM); LAPM follows Caltrans

City of Long Beach OAP

Construction Manual filing category requirements. See Attachment #6 (Section 16.3 "Project Records" of the Local Assistance Procedures Manual , 2021 ).

✓ The complete project file be available at a specific location at the Agency for inspection by Caltrans and Federal Highway Administration (FHWA) personnel.

✓ The project files shall be available for at least three years following the date of final project voucher.

✓ The use of a "Log Summary," as shown in Attachment 16Z2 of the LAPM Manual, facilitates reviews of material sampling and testing by Caltrans and FHWA, and assists the Project Engineer in tracking the frequency of testing.

✓ Project files will be maintained and available for at least three years.

When two or more projects are being furnished identical materials simultaneously from the same plant, it is not necessary to take separate samples or perform separate tests for each project; however, copies of the test reports are to be provided for each of the projects to complete the records.

5-102B Indexing

Use a category index, similar to the sample shown at the end of this section, or an index of categories that is supplied with the labels, for each project. Post the index in a prominent location.

When the location of a category is separate from the main file, indicate its location on the index under the appropriate heading.

5-102C Description of Categories

The following discussion describes the documents that should be included in each category and , for some categories, a recommended order of the documents in the categories:

Category 1 Project Personnel

Include all personnel-related records in this category. Suggested subcategories are listed below. On smaller projects, some of the listed subcategories may be combined when the amount of detail shown is not warranted.

• Form CEM-0101, "Resident Engineer's Report of Assignment"

• Attendance Report

• Overtime Records

• Overtime Requests and Authorizations

• Absence Requests

• Personnel Transfer Records

• Personnel Roster

• Travel Expense Claims and Records

• Individual Personnel File (emergency telephone numbers, experience or training records, among other things)

Category 2 Project Office Equipment and Supplies

In this category, file those documents re lating to equipment and supplies. Include records of equipment and supplies that have been received or returned. The subcategories listed below outline the scope of this category.

• Equipment Inventory

• Shipping Records (related shipping and receiving records should be stapled together)

• Receiving Records

• Transfer Requests

• Local Requests

• Automotive Records

Cash Expenditure Vouchers

Purchase Orders

Bills of Lading

Category 3 Equipment and Personnel Cast Reports

In this category, file construction engineering cost reports. Suggested subcategories are:

PRSM (task management) reports

Equipment cost reports

Category 4 Service Contracts

In this category, file documents related to the project office utilities and services. File requests for service with all correspondence relating to project office service contracts in an appropriate subcategory. File the receiving records for bills for utilities and services In a "date received" sequence.

It is recommended that a separate subcategory be used for each company or each service agreement. File purchase orders for supplies in Category 2, "Project Office Equipment and Supplies."

The subcategories that may be included in this category are as follows:

Rent

Electricity

Gas

Telephone

Drinking water

Overnight mail and shipping service

Additional service agreements, as required

Do not confuse this category with Category 16, "Utility Agreements," Category 17, "Utility Work Performed,'' or a subcategory of Category 52, "Charges to Total Contract Allotment." These are part of the project's construction operations.

Category 4 includes only those transactions connected with the resident engineer's office.

Category 5 General Correspondence

In this category, file letters that do not relate to any other category or subcategory in use. File correspondence concerning a subject that directly relates to some other category In that category. For example, file correspondence developed in connection with a change order in the change order category file.

File correspondence in any subcategory in chronological order.

California Department of Transportation. Construction Manual. July 2019 Project Records and Reports

When the volume of correspondence builds up, segregate and divide it into more detailed subject subcategories. When appropriate, transfer correspondence from Category 5 to a more specific category. For example, a property owner may object to certain conditions on the project. After considerable correspondence, the resident engineer writes a change order to solve the problem. At this point, the resident engineer should transfer all of the correspondence related to the change order to the change order category file.

A letter might cover subjects in multiple categories. When the letter relates directly to two subjects, file a copy in each category or cross-reference to the location of the original. Cross-referencing need be only a note describing the letter filed in a different category.

The following are examples of subcategories in Category 5. The number of subcategories will depend on the volume of correspondence. Show all subcategories in the index.
