About this file
This is an Offeror Response Form (Exhibit B) for RFP AADA 2027-0046, issued by a state agency's Department of Public Assistance (DPA) for ACP Client Data Removal Services. The solicitation seeks qualified vendors to provide comprehensive data removal and opt-out services for ACP (Affordable Connectivity Program) participants, including automated and manual removal of personally identifiable information from public people search engines, data brokers, and online registries. Offerors must demonstrate experience within the past five years providing these services with high success rates and secure PII handling capabilities. The response form requires detailed information across five primary categories: organizational experience and qualifications, operational capabilities and procedures, personnel resources and staffing plans, alignment with the Statement of Work (Exhibit A), and cost proposals to be submitted via Exhibit C - Pricing Worksheet.
The operational requirements outlined in the response form include specifications for the number of data brokers and websites covered, capability to remove data from international sources, subscription terms, rescanning frequency, data types targeted for removal, security and compliance certifications, and methods of execution (automated, manual, or hybrid). Offerors must explain their processes for obtaining legal authority to act on behalf of subscribers, handling non-responsive removal requests, protecting subscriber data, and providing aggregate utilization reports. Personnel requirements mandate identification of key personnel, staffing levels with hourly dedication, succession planning, and comprehensive subcontractor utilization plans if applicable. The Department will cover all service costs directly to eliminate expenses for ACP Participants, and offerors must confirm their ability to bill the Department while working directly with participants on service delivery.
