About this file
This is an Insurance Requirements exhibit (Exhibit A) document for the Southwest Storage and Repump Facility project managed by Volusia County, Florida, in coordination with the Florida Department of Transportation (FDOT). The document establishes comprehensive insurance coverage mandates for all contractors, subcontractors, and independent contractors participating in the construction of a two-million-gallon potable water storage tank, high-service pump station, and combined electrical/IT/chemical storage building. The project involves civil site development, concrete foundations, mechanical systems, electrical and instrumentation installations, requiring full compliance with Volusia County Utilities standards and applicable permit requirements.
The insurance requirements mandate Workers Compensation with a Waiver of Subrogation in favor of the County and FDOT, Commercial General Liability on an occurrence basis with limits of $1,000,000 per occurrence and $2,000,000 general aggregate, and Auto Liability with a $1,000,000 combined single limit. Additional coverage requirements include $5,000,000 in Excess Liability on a follow-form basis, Contractors Pollution Liability at $2,000,000 per incident and aggregate, Professional Liability at $1,000,000 per claim and aggregate, and Builder's Risk coverage with the County as loss payee. All policies must name the County and FDOT as additional insureds on a primary and non-contributory basis, include Waivers of Subrogation, prohibit self-insurance and policy retentions, and require thirty days' written notice of cancellation to the Certificate Holder at Volusia County Purchasing & Contracts Division in DeLand, Florida.
