About this file

This is an Exhibit B Deliverables document outlining the submission requirements for a hospital laundry services contract. The contractor must submit deliverables across three categories: one-time submissions at proposal and contract award, monthly recurring submissions, and as-required submissions upon request or upon change.

One-time deliverables include a statement of company experience with hospital laundry services, a phase-in/transition plan due within five calendar days of contract award, and samples with manufacturer specifications for all proposed textile items. Monthly deliverables require submission of a Quality Control Inspection Report by the 5th calendar day of each month summarizing inspection results and quality improvement actions. As-required deliverables include designation of a Project Manager and alternate within ten days of award, an initial personnel roster 15 days prior to contract start for all staff handling textile pickup and delivery, health certificates and immunization documentation, an Exposure Control Plan, textile testing lab results within five work days of receipt, textile retest results upon receipt, laundry wash formulas within 15 days of contract start, a contractor safety plan, employee training records, a Contingency Plan within 15 days of award, and packing slips for each cart delivered identifying contents by item count. All deliverables are submitted to the Contracting Officer (KO) and Contracting Officer Representative (COR) with varying submission deadlines tied to contract milestones or operational requirements.
