About this file

This is a Vendor Disclosure Statement Form required by the State of Colorado under Colorado Revised Statute 24-102-206 for all state contracts and solicitations for services executed after August 3, 2007. The form applies to contracts where services may be performed outside the United States or the State of Colorado. Vendors must disclose the geographic locations where contract services, including any subcontract work, will be performed and provide justification for performing work outside U.S. or Colorado boundaries. The form requires vendors to identify specific countries and/or states where services will be performed and explain the necessity or advantage of conducting work in those locations.

The disclosure requirement does not apply to contracts under Medicare, the Colorado Medical Assistance Act, the Children's Basic Health Plan Act, or the Colorado Indigent Care Program. Vendors must provide their name, the contract routing number, and complete the disclosure form only if services are anticipated to be performed outside the United States or Colorado; completion is waived if all work will be performed within Colorado or the United States.
