About this file
This is a vendor certification attachment for the Energy Assistance Program (EAP) Intake Site Application Assistance contract opportunity issued by the State of Nevada, Department of Human Services, Division of Social Services. The certification document establishes compliance requirements for contract recipients regarding the employment of current and former State employees under Nevada Revised Statutes 333.705. Contract recipients must disclose any employees who are current State employees or were employed by the State within the preceding 24 months, and must obtain Agency approval before utilizing such employees to perform services under the contract. These restrictions apply equally to any subcontractors engaged to perform contract requirements.
The certification prohibits the use of current State employees and former State employees without prior notification to and written approval from the issuing Agency, with the exception of former State employees who are not receiving retirement benefits under the Public Employees' Retirement System (PERS) during the contract duration. The document requires the vendor to execute the certification by providing the company name, printed name, title, signature, and date of the authorizing representative, confirming the vendor's agreement to comply with all stated provisions.
