About this file

This is a Disqualification Disclosure form required by the City of Pasadena's Department of Water and Power (PWP) in accordance with California Public Contract Code Section 10162 for the Distributed Energy Resource - Rooftop Wind Microturbine Project. The form requires prospective bidders to complete a questionnaire under penalty of perjury regarding any disqualification, removal, or prevention from bidding on or completing federal, state, or local government projects due to violations of law or safety regulations. Bidders must indicate whether they, any officer of the firm, or any employee with a proprietary interest in the business have experienced such disqualification. If a bidder answers affirmatively, they are required to explain the circumstances and attach supporting documentation.

This disclosure requirement is a standard qualification criterion for the proposed project, which seeks proposals for the design, engineering, permitting, procurement of materials, writing of construction bid requests, and maintenance of a rooftop wind turbine with a minimum capacity of 5 kilowatts to be installed on a municipally owned building at the City Maintenance Yards. The solicitation aims to select a private entity to develop this distributed energy resource for operation by PWP, with proposals required to include the purchase price of the wind turbine unit and associated engineering costs, while construction will be subcontracted separately by the City.
