Design-Build Team Reference Check Instructions, Form, and Sample Letter

This document is a reference check instruction guide and evaluation form prepared by the City of Phoenix Office of the City Engineer's Design and Construction Procurement section for the Road Safety Action Plan Safe Streets and Roads for All Fiscal Year 2024 Corridor Study project. The file provides standardized instructions, a performance evaluation form template, and a sample cover letter that design-build teams must use when requesting references from previous clients. Responding firms must submit completed reference evaluation forms from the first three references to soq.referencechecks@phoenix.gov by the specified deadline (12:00 p.m. Phoenix time on the deadline date listed in individual RFQs). A mandatory pre-submittal meeting is scheduled for January 29, 2026, at 1:00 p.m., with Statements of Qualifications due by 12:00 p.m. Phoenix time on Friday, February 13, 2026, through the City's Procurement Portal. The reference checks are a formal part of the evaluation process, with each of the first three references worth up to 7 points for a maximum total of 21 points toward the overall scoring.

The reference check form requires evaluation of design-build team performance across seven rating categories: business relationship quality, team consistency from proposal to completion, problem-solving proactivity, on-time and on-budget delivery, work quality, innovation and efficiency recommendations, and willingness to contract again. Each category is scored on a 4-point scale (Unsatisfactory at 0.25 points, Poor at 0.50 points, Good at 0.75 points, and Excellent at 1.0 point), with comments optional but encouraged. The completed forms become public record and may be released to competing firms upon request. References must be submitted directly by the owner or owner's representative responsible for project oversight, with the completed evaluation forms establishing a portion of the 1,021 total evaluation points available for the project. The City reserves the right to reject any or all Statements of Qualifications and will proceed with qualifications-based selection and negotiation with the highest-ranked firm.
