About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on July 20, 2017. The document serves as an informational guide for potential bidders on construction projects and outlines common bidding requirements and disqualification criteria. The form emphasizes that all potential bidders must thoroughly review the Request for Quotation (RFQ), all associated documents, and related instructions in their entirety to understand specific project requirements, as the form itself does not constitute a complete list of bidding requirements for any individual construction project.
The form identifies twelve errors that result in immediate bid disqualification, including failure to attend mandatory pre-bid meetings, failure to sign required bidder instruction pages, failure to provide a West Virginia contractor's license number, failure to submit a signed drug-free workplace affidavit, failure to provide a valid bid bond, failure to meet mandatory RFQ requirements, failure to acknowledge receipt of Addenda if required, failure to submit bids by the deadline, and debarment or suspension by federal or state authorities. Additionally, the form lists three errors that may result in bid disqualification prior to contract award if unresolved: uncontested debt to the State exceeding $1,000, Workers' Compensation or Unemployment Compensation delinquency, and failure to obtain required bonds and insurance. Contractors are cautioned that this list, while addressing common problems in the construction bidding process, is not exhaustive and does not address every potential grounds for disqualification.
