About this file
This is a Construction Bid Submission Review Form issued by the State of West Virginia Department of Environmental Protection (DEP) for the Buffalo Creek (Slone) Portals project (ARFQ DEP2100000062). The form was created on August 11, 2017, and serves as an informational guide for potential bidders regarding common bidding requirements and disqualification criteria for construction projects. While not an exhaustive list of all requirements, the document emphasizes that all potential bidders must thoroughly review the complete Request for Quotation (RFQ) and all related bid documents in their entirety to identify actual requirements and avoid disqualification. The form outlines that failure to comply with stated requirements may result in bid disqualification, and bidders are responsible for understanding all conditions prior to submission.
The document identifies ten errors that result in immediate bid disqualification, including failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply valid bid bonds, failure to meet mandatory RFQ requirements, failure to acknowledge addenda when required, late bid submission, federal or state debarment, and outstanding environmental enforcement actions or unresolved DEP vendor complaints. Additionally, five categories of errors may result in bid disqualification before contract award, including unresolved debt to the state or political subdivision, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to supply a West Virginia contractor's license number within one day of request, failure to submit a signed drug-free workplace affidavit within one day of request, and failure to provide a subcontractor listing within one business day of bid opening or DEP request.
