About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017, designed to inform potential bidders of common disqualification issues in the construction bidding process. The form serves as an informational guide rather than a comprehensive list of all bidding requirements and does not replace the need for bidders to thoroughly review all bid documents, requests for quotation, addenda, and related instructions in their entirety. Bidders are cautioned that failure to comply with stated requirements may result in automatic bid disqualification, and the form emphasizes that all potential bidders must read bid documents completely to identify actual requirements and avoid oversights.
The form identifies two categories of disqualifying errors: immediate bid disqualification offenses and errors that may result in disqualification before contract award. Immediate disqualifiers include failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply valid bid bonds, non-compliance with mandatory RFQ requirements, failure to acknowledge receipt of addenda when required, late bid submissions, federal or state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Errors that may result in disqualification before award include outstanding debt to the state or political subdivisions, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to supply a West Virginia contractor's license number within one day of request, failure to provide a signed drug-free workplace affidavit within one day of request, and failure to provide a subcontractor listing within one business day of bid opening or DEP request.
