About this file
This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The form serves as an informational guide for potential bidders on construction projects and outlines common problems that may result in bid disqualification. The document emphasizes that all bidders must thoroughly review the complete Request for Quotation (RFQ) and all related bid documents to identify specific requirements for individual construction projects. The form is not intended as a comprehensive list of all bidding requirements but rather highlights the most frequently encountered issues in the construction bidding process.
The form identifies two categories of disqualifying errors. Immediate disqualification errors include failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply valid bid bonds, failure to meet mandatory RFQ requirements, failure to acknowledge addenda (if required), late bid submission, federal debarment, West Virginia debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Errors that may result in disqualification before contract award include outstanding debt to the state or political subdivisions, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to supply a West Virginia contractor's license number within one day of request, failure to supply a signed drug-free workplace affidavit within one day of request, and failure to provide subcontractor listings within one business day of bid opening or DEP request.
