About this file
This document outlines procedures for project commissioning, closeout, and transition for the Defense Information Systems Agency. It details the process for transitioning systems, equipment, and components added or modified under a project to base operations support and maintenance contracts. This includes providing updated equipment inventories, operation and maintenance documentation, warranties, and training facility managers and maintenance personnel. It requires label layouts during design, inspections and punch lists upon work completion, testing and demonstration at substantial completion, and final as-built drawings and manuals for contract closeout.

PROJECT COMMISSIONING, CLOSE-OUT, AND TRANSITION
This document outlines procedures to ensure positive control of vital project functions and information, from Notice to Proceed through warranty expiration, to ensure timely and seamless lifecycle management of DISA Systems, Equipment, and Components (SEC).
Transition of SEC to Base Operation Support (BOS)/Operations and Maintenance (O&M) through the commissioning and close-out process requires that all detailed SEC installation information be provided to the BOS Maintenance team to add to the appropriate service and maintenance contract(s). To effectively transition the information for lifecycle service and maintenance the following tasks shall be completed on all projects (as applicable):
1) 65% Design: The contractor shall submit label layouts for all added, modified, or altered SEC to the project engineer for BOS Section review and approval.
2) Completion of Physical Work:
a. The project engineer and facility manager completes an on-site walk-through of the completed work as a final QA/QC of the project. The BOS Section shall attend at their discretion.
b. The contractor develops a punch list identifying minor adjustments and repairs necessary prior to financial close-out and submits the punch list to the project engineer.
3) Substantial Completion:
a. The contractor shall provide an update to the SEC Inventory to include all SEC added, modified, altered, or removed. Submission will be through the project engineer to the BOS Section.
b. The project engineer shall ensure commissioning (installation, performance, functional performance testing, integrated system testing, and documentation) of all systems, equipment, and components for proper function. Attendees at this commissioning and testing shall include the contractor, subs/manufacturer reps, project engineer, facility manager, and site maintenance personnel (all as applicable). The BOS Section shall attend at their discretion.
c. After completion of final commissioning (and at the discretion of the BOS Section), the project engineer shall demonstrate all system functions, sequences and operations to the BOS Section.
d. The contractor shall provide details, terms, and conditions of all SEC warranties through the project engineer to the BOS Section to ensure seamless transition from the end of the warranty period to start of O&M service and maintenance support.
e. The BOS Section initiates modification of applicable service and maintenance contracts.
4) Contract Close-Out:
a. The contractor shall provide Final As-built documents (dwg and specs) to the project engineer.
The project engineer shall provide a copy to the BOS Section and the host installation.
b. The contractor shall provide final O&M Binders (O&M Manuals, Factory Tests, Commissioning documents, Operating information, SEC Inventory, Warranty documents, punch list, etc.) through the project engineer to the BOS Section and facility manager.
c. The contractor shall provide spare parts, extra materials, and keys through the project engineer to the facility manager.
d. The project engineer shall ensure all O&M training is completed by the facility manager and site maintenance personnel.
e. Contract site maintenance personnel shall submit a letter of acknowledgement of training to the BOS Section within 30 days of training.
f. The contractor shall submit all information for capital funds close-out through the project engineer to the ES622 Funds Manager.
5) Post Contract Close-Out:
a. The project engineer will receive, sign, and file DD Forms 250 (Material Inspection and
Receiving Report) and 1354 (Transfer and Acceptance of DoD Real Property).
b. The project engineer will update the site Infrastructure Study with current configuration data.
c. Upon BOS Section acceptance of this checklist, the affected SEC shall fully transition from the project engineer to the BOS Section for lifecycle service and maintenance.
d. The BOS Section ensures revision update of Master Drawings, based on submitted As-builts.
