About this file

This document is a Standard Operating Procedure (SOP) for cervical cancer screening at the VA North Texas Health Care System's Ambulatory Care Service, effective December 18, 2020, with recertification scheduled for December 18, 2025.

The SOP establishes protocols for offering cervical cancer screening to women Veterans in accordance with U.S. Preventive Services Task Force (USPSTF) and Department of Veterans Affairs guidelines. Key procedures include assigning new enrolling women Veterans to a Designated Women's Health Primary Care Provider (DWHP), requiring female chaperones during examinations, and mandating provider responsibility for tracking and timely follow-up of screening findings. Providers must utilize Clinical Reminders within the Computerized Patient Record System (CPRS) and databases such as VSSC or DataMart to track screening completion. The Pathology and Laboratory Medicine Service notifies providers via CPRS View Alert when results are available, and providers must communicate results according to VANTHCS Memorandum 11-14. The Women's Health Cancer Prevention Coordinator provides care coordination and follow-up for abnormal results, ensures quality assurance through result tracking and notification timeliness, and reports monthly data to the Women Veteran Health Committee. Appendix A provides screening guidelines specifying that average-risk women should begin screening at age 21, with cytology screening every 3 years for ages 21-65 and hrHPV co-testing every 5 years for ages 30-65. Screening is not recommended for women under 21 or over 65 with adequate prior screening and normal results, or for those with total hysterectomy without cervical cancer history.

For women age 30-65 hrHPV co-testing every 5 years is recommended.

Women aged over 65 years with adequate recent screening and normal CCS results are not otherwise at high risk for cervical cancer may discontinue Pap testing.

*NOTE: if utilizing hrHPV testing alone every 5 years, the clinician must ensure that the testing used has been FDA approved specifically for this purpose.

These recommendations are congruent with the recommendations from U.S. Preventive Services Task Force Recommendation Statement.1–4

Negative screening tests may not be definitive to rule out any type of cancer.

These recommendations do not apply to individuals who have been diagnosed with a high-grade precancerous cervical lesion or cervical cancer. These recommendations also do not apply to individuals with in utero exposure to diethylstilbestrol (DES) or those who have a compromised immune system (e.g., women living with HIV).1 These women are at increased risk and require individualized follow-up.
