About this file
This is Amendment 0006 to Solicitation 36C25726R0018, issued by the Department of Veterans Affairs, VHA RPO West, Health Care Resources Branch on August 20, 2026. The amendment's purpose is to post questions and answers related to the underlying solicitation. The solicitation amendment extends the hour and date specified for receipt of offers, and offerors must acknowledge receipt prior to the deadline by completing the amendment form, acknowledging receipt on offer copies, or providing separate written communication.
The document contains four question-and-answer pairs clarifying requirements in the Performance Work Statement (PWS) for what appears to be a primary care and mental health services contract at VA facilities. Key clarifications include: (1) mental health administrative-support costs shall be included in the Primary Care PMPM rate, with estimated workload data referenced in PWS Section 5.5; (2) dual-purpose space is permitted whereby the mental health group room may serve as a tele physical therapy space; (3) the Government will furnish mental health injectables prescribed by VA Mental Health providers through VA Pharmacy, and provided historical utilization data for eight medication types at the Sherman CBOC and current LAI medications utilized at VA North Texas; and (4) offerors may submit alternate pricing approaches to recover construction and build-out costs outside the Base Year PMPM rate (superseding a previous answer), but must include both a fully compliant Base Year PMPM approach incorporating all costs into CLIN 0001 and a distinct alternate pricing option with supporting rationale for Government evaluation.

Q121. PWS Sections 2.3, 2.4, 2.6, and 2.7 state that the VA will provide discipline-specific PACT staff, specialty-care staff, General and Specialty Mental Health clinicians, and telehealth clinical staff, while the contractor must provide adequate space and support personnel. For Mental Health specifically, please provide the current number of Veterans receiving General and Specialty Mental Health services and the anticipated number at the beginning of the Base Year. This information is necessary because PWS 2.6 requires one administrative-support FTE per 1,000 Veterans receiving these services. The pricing schedule contains only a Primary Care PMPM LIN, although the PWS definition of PMPM refers to both Primary Care and Mental Health LINs. Please confirm whether the Mental Health administrative-support costs must be included in the Primary Care PMPM rate or whether a separate Mental Health LIN was inadvertently omitted.
A121. The offeror is required to provide clerical support services for mental health as outlined in section 5.5. Mental Health Services, page 40, in the paragraph titled Mental Health Support Services. This service will be included in the Primary Care PMPM rate. To estimate the number of enrollees receiving general and specialty services, please refer to section 5.5 Mental Health Services, Estimated Mental Health Workload, on page 37.
Q122. Would the VA consider allowing the mental health group room or group appointment room to be used as a dual-purpose space for tele physical therapy, given that the RFP does not identify a dedicated tele physical therapy room and the 200 square foot requirement stated in Amendment 0003 appears to be the only stated space requirement for this service?
A122. Yes, the mental health group room or group appointment room may be used as a dual-purpose space for tele physical therapy.
Q123. In Amendment 0004 question 109, the question was asked, will the Government confirm contractor is only responsible for medications within their scope of practice, this would exclude mental health and other specialty care injectables? And the VA's response was, Mental health injectables as noted on the VANTHCS PACT Medication list provided are the contractor's responsibility. Will the VA provide historical utilization names of medications and quantities of mental health injectables for the prior 12 months?
A123.
Please see number of patient medication orders provided to the Sherman CBOC for the last 12 months per medication.
• Aripiprazole 400mg injection - Two patient orders
• Fluphenazine 25mg/ml - Two patient orders
• Haloperidol 100mg/ml - One patient order
• Paliperidone 39 mg, 78 mg, 156 mg, 234 mg, 410 mg/kit, 546 mg/kit, 819 mg/kit - Eleven patient orders (among those doses)
• Naltrexone 380mg - Ten patient orders
Government-furnish mental health injectables prescribed by VA Mental Health providers through VA Pharmacy;
Current LAIs utilized at VA North Texas include:
Aripiprazole (Abilify Maintena) Aripiprazole (Aristada Initio) Aripiprazole lauroxil (Aristada) Fluphenazine (Prolixin) Haloperidol (Haldol) Paliperidone (Invega Sustenna, Invega Trinza) Risperidone (Risperdal Consta) Risperidone (Perseris) Naltrexone (Vivitrol)
Q124. As a follow-up to Q&A #100 of Amendment 0003 which confirms that construction, build-out, start-up, and associated facility costs must be incorporated into the Base Year PMPM rate, please confirm whether an offeror may submit more than one independently compliant facility alternative within a single proposal submission, with each alternative accompanied by its own complete Schedule of Services/PMPM pricing. Each alternative would independently satisfy the solicitation requirements and incorporate its respective facility costs into its Base Year PMPM rate. If permitted, may common portions of the Technical Capability response apply to both alternatives, with only the facility-specific technical approach and pricing separately identified for each?
A124. This response supersedes A100.
Although Section E of the solicitation states that build-out and start-up costs, if needed, shall be calculated into CLIN 0001, the Government confirms that offerors may propose an alternate pricing approach. Specifically, offerors may submit:
• A separate construction CLIN,
• A progress-payment structure,
• An alternate build-out price, or
• Any other reasonable method of recovering these costs outside the Base Year PMPM rate.
Any alternate pricing proposal must be clearly identified, fully supported, and separately structured so the Government can evaluate price reasonableness and compliance with solicitation requirements.
Offerors choosing to propose an alternative method shall include both:
1. A fully compliant Base Year PMPM approach that incorporates all costs into CLIN 0001, and
2. A distinct alternate pricing option detailing the proposed cost-recovery method and supporting rationale.
Both approaches will be evaluated in accordance with the solicitation’s best-value procedures.
