About this file
This file is a Post Questions and Answers amendment to solicitation 36C25726R0018, issued by the Department of Veterans Affairs Veterans Health Administration on June 29, 2026, with an effective date of July 2, 2026. The amendment addresses three clarifications regarding proposal submission requirements for the VA North Texas Health Care System's community-based outpatient clinic (CBOC) services contract.
The three questions and answers clarify evaluation factors: (1) offerors may submit an unlimited number of Past Performance Questionnaire Worksheets (Attachment D.21) to adequately demonstrate relevant past performance, with no limit imposed; (2) offerors are responsible for completing all 12 questions in the Past Performance Questionnaire Worksheet (Attachment D.22) for submission with their proposal, with the VA reserving the right to contact references for additional information or clarification; and (3) offerors must address each section of the Performance Work Statement individually when describing their methodology under Part II Factor 1 - Technical Capability, providing detailed explanations of how they will meet each PWS section's requirements. All other terms and conditions of the original solicitation remain unchanged and in full force and effect.

CONTINUATION PAGE
Q1. In the General Instructions for Proposal Submission, under Factor 2 - Past Performance on page 97, it states that offerors shall complete Attachment D.21 Past Performance Questionnaire Worksheet (RFP 36C25726R0018). How many Past Performance Questionnaire Worksheets would the VA like the offeror to submit?
A1. There is no limit. Offerors may submit as many Past Performance Questionnaire Worksheets as they feel are necessary to adequately demonstrate their relevant past performance.
Q2. Attachment D.22 Past Performance Questionnaire Worksheet appears to include questions intended for different respondents. For example, several items request factual contract information typically provided by the offeror, while Question 3 appears to request an assessment from the Past Performance reference. Will the VA clarify whether the offeror is expected to complete all 12 questions and submit with the proposal, or if certain questions are intended to be completed directly by the Past Performance reference? If the latter, will the VA identify which questions should be completed by the offeror versus the reference?
A2. Yes, the offeror is expected to complete all 12 questions and submit the completed questionnaire with their proposal. If needed, the VA will contact the Past Performance reference identified on the questionnaire for additional information or clarification.
Q3. In the General Instructions for Proposal Submission on page 97, Part II Factor 1 - Technical Capability states that the offeror shall describe in detail the methodology to accomplish the requirements of each section of the Performance Work Statement (PWS). Given the breadth of the PWS, the potential length of responses, and the time required for evaluation, was it the VA’s intent for offerors to address each PWS section individually? If not, will the VA clarify the specific information it would like offerors to address under Part II Factor 1 - Technical Capability (e.g., QA Plan, experience, organizational structure, staffing plan, transition plan, facility suitability/accessibility, etc.)?
A3. Yes, the VA’s intent is for the offeror to address each section of the PWS individually when describing their methodology under Part II Factor 1 – Technical Capability. The offeror should provide a detailed explanation of how they will meet the requirements of each PWS section as written. The VA will evaluate the offeror’s full methodological approach based on the information submitted.
