About this file

This is an amended Roster of Firms (ROF) procurement document for the Maryland Division of Rehabilitation Services (DORS) related to Contract CS #26-05, the Vehicle Equipment Adaptation Prescription Agreement (VEAPA). The procurement seeks qualified contractors to furnish and install adaptive equipment for DORS clients with functional limitations due to disabilities. The document reflects changes made following a pre-bid meeting and establishes a roster-based contracting approach rather than a single award. The contract is intended to provide on-demand vehicle adaptation services tailored to individual client needs, with services ranging from equipment selection through professional installation to ensure compliance with disability accommodation requirements.

The amended ROF document outlines the framework for multiple qualified firms to be added to an approved roster, enabling DORS to issue task orders as client needs arise. Pricing terms are to be provided by responding firms as part of their proposal submissions, with costs varying based on specific adaptive equipment requirements and installation complexity for individual clients. The roster structure allows for flexible contracting without predetermined volume commitments, accommodating the variable nature of client requests. Firms selected for the roster must demonstrate experience in vehicle adaptation services, proper licensing, and ability to meet DORS specifications and quality standards. This approach provides DORS cost efficiency through competitive procurement while ensuring timely access to qualified service providers for eligible clients requiring vehicle modifications.

Maryland State Department of Education Division of Rehabilitation Services Vehicle Equipment Adaptation Prescription and Agreement (VEAPA and Bid Form)

ATTACHMENT B
VEAPA WORKBOOK AND BID FORM INSTRUCTIONS
This Excel Workbook contains a list of prescribed vehicle equipment as well as the Bid and Signature Form. The different worksheets can be accessed by clicking the different Tabs at the bottom of this Workbook.
The Bidder is to provide pricing for "Equipment Cost" and "Labor Cost" in the appropriate cells to the right of each piece of prescribed equipment. Those are the only numbers that are required by the Bidder.
Bidders should not calculate any Total Costs.
Excel automatically adds and totals each section and then populates those calculations and totals to the last form in the Workbook; which is the "Bid Form & Signatures" Tab.
Bidders should not "un-protect" the Workbook or Worksheets within the Workbook.
Do not alter the Bid Form or the Bid Form may be rejected.
The Bid Form is to be signed and dated, where requested, by an individual who is authorized to bind the Bidder to the prices entered on the Bid form.
Bidders must be NMEDA QAP/MQAP certified to be awarded a vehicle modification contract with DORS.
If Bidder has difficulty with this Excel Workbook contact Amanda Perkins by email at Amanda.Perkins@maryland.gov for assistance with the Workbook.
If Bidder has questions or needs clarification on specifications prescribed in this VEAPA please email them to Jeremy Wilson at jeremy.wilson@maryland.gov and copy Brittany Owens at brittany.owens1@maryland.gov at Maryland State Department of Education (MSDE) procurement. All inquiries must be made in writing via email.
