About this file
This is a Conflict of Interest Affidavit document related to the Maryland Division of Rehabilitation Services (DORS) Vehicle Equipment Adaptation Prescription Agreement (VEAPA) procurement for client CS #26-05 (Contract R00B6600195). The affidavit establishes the conflict of interest declaration requirements for bidders and contractors responding to this vehicle adaptation services opportunity. The document outlines the mandatory certification that prospective contractors must complete and submit as part of their bid package, confirming they have disclosed any potential conflicts of interest related to the provision of vehicle adaptation services, including furnishing and installing adaptive equipment for DORS clients with functional limitations due to disabilities.
The affidavit serves as a compliance and accountability mechanism within Maryland's procurement process, requiring all bidders to certify their understanding of conflict of interest regulations and their commitment to transparent disclosure. Contractors must affirm that they have no undisclosed financial interests, relationships, or circumstances that would create conflicts with their ability to perform vehicle adaptation services impartially and in the best interest of the DORS client. This document is a prerequisite for bid submission and demonstrates adherence to state procurement ethics standards and regulatory requirements governing contractor eligibility.
