About this file
This is a Cost Proposal form for Request for Proposal Number 126746 O3, issued by the State of Nebraska Department of Health and Human Services (DHHS) to conduct a comprehensive Market Rate Survey (MRS) of licensed childcare providers. The project is structured in five key deliverable phases: Phase 1 (Planning) involves establishing an advisory committee and developing a draft survey for DHHS approval; Phase 2 (Data Collection) requires collecting data using approved methods; Phase 3 (Data Analysis) focuses on generating preliminary rate estimates categorized by geographic location, provider type, and children's age groups; Phase 4 (Report Writing) involves preparing a comprehensive report with recommendations; and Phase 5 (Final Report and Presentation) requires delivery of executive summary findings and recommendations in both print and electronic formats to DHHS and the Early Childhood Interagency Coordinating Council. The initial contract term spans four years (2026-2029), with an optional one-year renewal period (2030-2033).
Bidders must complete the cost sheet by providing pricing for each deliverable category for all four years of the initial term and the optional renewal year. The pricing evaluation and final contract award will be based on the total cost of the initial four-year term plus the optional one-year renewal period. The cost sheet maintains a fixed format that cannot be altered, with unit prices governing in cases of mathematical errors in price extensions. Bidders must submit this completed form as part of their Request for Proposal response package.
