About this file
This is a Contractor's Acknowledgment form issued by Clay County Utility Authority (CCUA) in Middleburg, Florida, required for the Omega Park CCUA Connection project (Invitation for Bid #25/26-007). The project involves utility improvements at Omega Community Park located at 4317 County Road 218 in Middleburg, including furnishing and installing approximately 190 linear feet of 8-inch PVC gravity sewer main, 335 linear feet of 2-inch water service, and 484 linear feet of 2-inch force main, along with new lift station construction, septic tank decommissioning, and site restoration. A mandatory pre-bid meeting is scheduled for October 30, 2025, at 1:00 PM at the park location, with bids due November 20, 2025, at 4:00 PM. The project requires substantial completion within 120 days from the Notice to Proceed and final completion within 150 calendar days.
The acknowledgment form mandates that contractors provide only materials compliant with CCUA's specifications, details, and Approved Materials Manual, with manholes designed according to CCUA Standard Details. Contractors are responsible for correction, removal, and replacement of all non-compliant items at their own expense. While the form serves as acknowledgment for standard materials listed in CCUA's Materials Manual, exceptions requiring individual shop drawings include alternate materials requested by the engineer, unlisted materials, and submittals for pumping stations and plant installations. Contractors must submit executed acknowledgments from each individual supplier before material installation. The total connection charges for the project amount to $27,670.30, which includes potable water capacity charge, alternative water supply surcharge, wastewater capacity charge, and environmental impact charges. Contractors must have been in business for a minimum of five years performing similar work and are required to provide a 5% bid bond and comprehensive insurance coverage including commercial general liability, business automobile liability, workers' compensation, and property coverage.
