About this file
This is a Material Supplier Acknowledgement Form issued by Clay County Utility Authority (CCUA) for the Omega Park CCUA Connection project located at 4317 County Road 218 in Middleburg, Florida. The form requires material suppliers and contractors to acknowledge and agree that they will provide only materials conforming to CCUA's specifications, details, and Approved Materials Manual. The project involves furnishing and installing new water and sewer mains, including approximately 190 linear feet of 8" PVC gravity sewer main, 335 linear feet of 2" water service, and 484 linear feet of 2" force main, along with constructing a new lift station and decommissioning existing septic tanks. A mandatory pre-bid meeting is scheduled for October 30, 2025, at 1:00 PM at the park, with bids due November 20, 2025, at 4:00 PM. The project requires substantial completion within 120 days from the Notice to Proceed and final completion within 150 calendar days. Contractors must have been in business for a minimum of five years performing similar work and must provide a 5% bid bond.
The form establishes CCUA's procedure whereby the Material Supplier Acknowledgement takes the place of individual shop drawings for standard materials listed in CCUA's Materials Manual, with exceptions for alternate materials requested by the engineer, unlisted materials, and materials associated with pumping stations and plant installations, which require individual shop drawing submissions for CCUA approval prior to installation. The design engineer retains the right to require additional submittals and shop drawings as deemed necessary. Total connection charges for the project amount to $27,670.30, which includes potable water capacity charges, alternative water supply surcharges, wastewater capacity charges, and environmental impact fees. Bidders must comply with comprehensive insurance requirements, including commercial general liability, business automobile liability, workers' compensation, and property coverage. Chen Moore and Associates is serving as the engineering design firm for the project.
