About this file

This is a Contractor Responsibility Form required by the City of Tucson, Arizona for the Blacklidge Drive Bicycle Boulevard–FHWA project. The Apparent Low Bidder must submit the completed form within four business days following bid opening and notification. The project involves constructing a bicycle boulevard along approximately 4.5 miles of Blacklidge Drive between Oracle Road (State Route 77) and Columbus Boulevard in Tucson, Pima County, Arizona. The bicycle boulevard will prioritize bicycling through traffic calming measures and will incorporate signalized pedestrian hybrid beacons (HAWK signals) to enable safer crossing for bicyclists and pedestrians without significantly impacting traffic flow. The HAWK signals operate in a yellow-red-flashing red sequence to alert motorists of crossing activity.

The form establishes responsibility criteria pursuant to A.R.S. Title 34, requiring contractors to demonstrate adequate financial resources, ability to meet delivery and performance schedules, satisfactory performance records, integrity and ethics, adequate experience and technical expertise, and access to necessary equipment. Contractors must certify whether they have been debarred or suspended from federal contracts, convicted of fraud or criminal offenses related to public contracts, or subject to civil judgments within the preceding three years. Additionally, contractors must disclose prior liquidated damages payments, terminations for poor performance, claims against bonds, legal actions regarding payment failures, and any anticipated bankruptcy or litigation that could materially impact project performance. Contractors are required to provide their Experience Modification rating and describe three representative similar projects with owner references, work descriptions, specialty subcontractor information, original and final project values including change orders, and original versus actual construction schedules. Contractors must also document current workload and resource availability to complete the project within the fixed delivery date.
