The form requires contractors to certify regarding debarment status, criminal convictions, and civil judgments within the preceding three years, and to provide detailed responses regarding prior contract performance including liquidated damages, project terminations, claims for improper work, payment disputes, and bonding claims. Contractors must submit current Experience Modification (EMod) ratings and describe three representative projects completed within the last three years that are similar in scope, including owner references, work descriptions, specialty subcontractor information, total project values with original and final amounts, original and actual construction schedules, and an explanation of any discrepancies. Additionally, contractors must disclose current workload and the percentage availability of resources and personnel intended for project completion by the fixed delivery date, and identify any pending legal actions or bankruptcy proceedings that could impact performance capability. Failure to provide sufficient supporting documentation or explanation may result in a non-responsibility determination and contract award denial.
