About this file

This compliance verification documentation from the State of Ohio outlines required documents that must be submitted with quotes for state contracts. The document specifies that compliance verification materials must be submitted separately from the bidder's certificate and quote, combined into a single document.

The required documentation includes a Certificate of Liability Insurance with endorsements and Stop Gap coverage, verification of current contact information, signed Bidder's Certification Form, Contractor/Subcontractor Affirmation and Disclosure Form regarding offshore services prohibition and Russian institution restrictions, Affirmative Action Program Verification Form, Secretary of State registration information, current BWC certification, and any applicable MBE, EDGE, VBE, WBE, or specialty trade certifications. The document emphasizes Ohio's prohibition on offshore services and investments in Russian institutions, requiring contractors to disclose service locations and affirm compliance with these restrictions.
