About this file
This document outlines compliance verification documentation requirements for Ohio state contract bidders. The document specifies that bidders must submit several required documents separately from their bidder's certificate and quote, including an Acord Certificate or Certificate of Liability Insurance with endorsements and Stop Gap coverage, current address verification, and a Contractor/Subcontractor Affirmation and Disclosure Form regarding offshore services restrictions.
The document emphasizes strict prohibitions on offshore services and Russian investments, requiring contractors to affirm they will not perform services outside the United States without state waiver or engage with Russian institutions. Additional required documentation includes an Affirmative Action Program Verification Form (AAPV), Secretary of State Charter/Registration Number, current BWC Certification, and any applicable MBE, EDGE, VBE, WBE certifications or specialty trade certifications as specified in the Scope of Work.
