About this file
This is a Safety Data Sheet template that offerors must complete to provide safety performance information for evaluation under a federal contract opportunity. The form requires submission of five complete calendar years of safety data (2020-2024) in the form of OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) rates and Total Recordable Case (TRC) rates, presented as decimal figures. Offerors must explain any missing data for specific years and provide detailed justifications for negative safety trends where DART rates exceed 3.0 or TRC rates exceed 4.5, including documentation of corrective actions implemented for improvement. Insurance carrier information, including the carrier name, agent contact details, and phone/email, must also be provided.
The Safety Data Sheet requires a technical approach narrative not to exceed two pages that addresses the offeror's Safety Management System implementation, including management and employee involvement, hazard prevention and control, worksite analysis, and safety training standards used for benchmarking. Additionally, offerors must describe their evaluation process for selecting subcontractors and detail the oversight processes for ensuring subcontractor safety compliance throughout contract performance, covering both subcontractors and in-house workforce. For joint ventures or partnerships, a Safety Data Sheet must be provided for each contractor, though only one consolidated safety narrative is required. DART and TRC rates should not be submitted for subcontractors.
