About this file

This is a Past Performance References template form for solicitation N0025326Q0008, used by offerors to document relevant contract experience and demonstrate capability to perform the work. The form requires offerors to list all major subcontractors and provide up to five detailed past performance references, each including contract number and title, prime or subcontractor designation, requirement descriptions, relevance to the current effort, and dollar value or percentage of similar work performed. For each reference, offerors must identify the contracting agency, contract type (such as IDIQ, FFP, CPFF, or Supply/Service), period of performance, maximum contract value at award with options, and final or current contract value. The form also requires primary points of contact for each reference, specifically the Program Manager and Contracting Officer, along with their commercial phone numbers and email addresses. Additionally, offerors may provide information regarding problems encountered on identified contracts and corrective actions taken, though they should not submit general performance information, as the Government will obtain such information directly from the listed references.
