About this file

This is a Safety Data Sheet template that offerors must complete to provide safety information for evaluation under the KB Dock Piers Ladder Replacement solicitation (N0025326Q0009) issued by Naval Undersea Warfare Center Division (NUWC) Keyport. The form requires submission of five consecutive calendar years (2020–2024) of OSHA safety metrics, specifically Days Away from Work, Restricted Duty, or Job Transfer (DART) rates and Total Recordable Case (TRC) rates, presented as decimal figures. Offerors must explain any missing data for specific years and provide detailed corrective action explanations for any negative trends, defined as DART rates of 3.0 or greater or TRC rates of 4.5 or greater. For Joint Ventures or partnerships, each contractor must submit individual safety data, though only one combined safety narrative is required. Subcontractors are explicitly excluded from DART and TRC rate submission requirements.

The Safety Data Sheet also requires a technical safety narrative not to exceed two pages, which must address the offeror's Safety Management System (SMS) implementation including management and employee involvement, hazard prevention and control, worksite analysis, and safety training standards. Additionally, offerors must describe their process for evaluating and selecting subcontractors and detail their oversight mechanisms for ensuring subcontractor safety compliance throughout contract performance. This safety evaluation component is part of the overall proposal submission for the pier ladder replacement work at NUWC Keyport, Washington and Naval Base Bangor, which is a 100% small business set-aside requiring FOB Destination pricing.
