About this file

This is a Past Performance References template form for RFQ N0025326Q0008 requesting offerors to document their relevant contract experience. The form requires offerors to list all proposed major subcontractors and provide up to five detailed past performance references. For each reference, offerors must identify the contract number and title, specify whether the work was performed as prime or subcontractor, describe the requirement and major subcontracts involved, and explain the contract's relevance to the current effort including the dollar value or percentage of similar work performed. Additional required information includes the contracting agency, contract type (such as IDIQ, BPA, FFP, CPFF, or CPIF), period of performance, maximum contract value at award including options, and final or current contract value actually awarded and funded.

The template requires offerors to provide primary points of contact for each reference, specifically the program manager and contracting officer names, commercial phone numbers, and email addresses. Offerors are instructed to include information regarding any problems encountered on the identified contracts and corrective actions taken to resolve those issues, though general performance information should not be provided as the Government will obtain such information directly from the references. This form serves as a critical evaluation tool for assessing offeror capability and past success on similar federal contracts.
