About this file

This is an attachment outlining the identification card and contractor credentialing process required for employees working on a Veterans Affairs renovation project (Project 620-20-105: Renovate 14AB for Homeless Program Phase 1).

Prior to commencing work, all contractor and subcontractor employees must complete a five-business-day credentialing process administered by the VA's Program Office Representative/Program Manager. The process requires the contractor's point of contact to submit a completed OF-306 Contractor Request Form and self-certification for each employee, transmitted securely to avoid exposing sensitive personal information such as social security numbers. Each contractor employee must then visit the nearest VA facility with the Fingerprinting Request Form and two forms of identification to submit fingerprints for processing. The WMC HROO PerSec Contractor Team screens all documentation and fingerprint results using the National Criminal History Check (NCHC) form to issue either a favorable or unfavorable screening decision before work authorization is granted. The document emphasizes that any substituted or newly added contract personnel must complete this full documentation and screening process prior to beginning work on the project. The effective date of this requirement is March 14, 2025 (Revision 04).

Notice of Identification Card Process and Required Contractor Documentation Project 620-20-105 Renovate 14AB for Homeless Program Phase 1 After award of the contract but prior to commencing work, each contract employee, including any subcontract employees, must work with their Program Office Representative/Program Manager identified in the contract to complete the following within five (5) business days of receiving these instructions.

STEP 1. The contractor point of contact (POC) shall submit completed Contractor Request Form, OF-306, and Self-Certification for each contractor employee listed on the contract to the Program Office representative/Program Manager. Please note, due to the personal information contained in the form, the information must be sent in a secure manner. Please do NOT e-mail a document containing social security numbers unless the e-mail is encrypted or the form is password protected.

STEP 2. Each contractor employee listed on the Contractor Request Form must take the Fingerprinting Request Form and two forms of ID to their nearest VA facility to have their fingerprints submitted and the form completed and submitted in accordance with the instructions on the form. It is imperative for each applicant to take a copy of the Fingerprinting Request Form to the fingerprinting facility as it determines which facility is billed for the fingerprint submission and where the results are sent. Contractors can contact the local Program Office Representative/Program Manager for assistance in scheduling an appointment.

WMC HROO PerSec Contractor Team VHAWMCPerSecContractor@va.gov screens all documents as well as the fingerprint results and provides the points of contact with a favorable or unfavorable screening decision for each contractor using the National Criminal History Check (NCHC) form.

Please Note: As contract personnel are substituted and/or added to the contract each individual must complete required documentation and receive a favorable screening decision prior to commencing work.
