About this file

This document is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania as part of state and local contract opportunities. The certification form mandates that contractors and grantees of the Commonwealth must comply with various Pennsylvania labor and workplace safety laws, including the Unemployment Compensation Law, Workers' Compensation Law, and other applicable state statutes. Contractors and grantees must certify that they are in full compliance with these laws, and failure to do so may result in contract termination and debarment from future contracts. The certification must be signed by an authorized official and submitted as part of the contract proposal.

The document outlines the specific state labor and workplace safety laws that contractors and grantees must comply with, and it emphasizes the penalties for providing false certifications, including contract termination and three-year ineligibility to bid on future contracts. The certification is subject to the provisions and penalties of Pennsylvania's Unsworn Falsification to Authorities statute.
