About this file
This document is a Small Business Subcontracting Plan Form template for SLAC National Accelerator Laboratory that large business subcontractors must complete for subcontracts exceeding the thresholds established in FAR 19.708.
The form requires subcontractors to identify the subcontract details (subcontractor name, address, solicitation number, item/service description, total amount including options, and performance end date) and select from three plan types: Individual Contract Plan, Master Plan, or Commercial Plan. Subcontractors must establish separate dollar and percentage goals for small business (SB), veteran-owned small business (VOB), service-disabled veteran-owned small business (SDVOB), HUBZone small business (HUB), small disadvantaged business (SDB), and women-owned small business (WOSB) concerns, as well as large business participation. The plan must include a description of principal subcontract types and the methodology used to develop goals, designation of a program administrator with contact information, and specification of whether indirect costs are included in goal calculations.
The form establishes duties and responsibilities including: ensuring equitable opportunity for small business concerns through outreach to trade associations, business development organizations, and procurement conferences; including flow-down clauses in all subcontracts offering further opportunities; requiring large business subcontractors with contracts exceeding $900,000 ($2,000,000 for construction) to adopt compliant subcontracting plans; and submitting periodic reports via SAM.gov. Required reporting includes Individual Subcontract Reports (ISR) and Summary Subcontract Reports (SSR) with due dates of May 15 and November 14 annually, along with Small Disadvantaged Business Reports if applicable. Subcontractors must maintain detailed recordkeeping documenting outreach efforts, solicitation records by contract, organizational contacts, and subcontract award data. Failure to file required reports timely and accurately constitutes a breach of contract terms, and SLAC reserves the right to delay invoice payment for such failures. SLAC's UEI number is HJD6G4D6TJY5, and questions should be directed to ncolley@slac.stanford.edu.

Federal Acquisition Regulation (FAR), paragraph 19.708(b) prescribes the use of the clause at FAR 52.219-9 entitled "Small Business Subcontracting Plan." The following is a suggested model for use when formulating such subcontracting plan. While this model plan has been designed to be consistent with FAR 52.219-9, other formats of a subcontracting plan may be acceptable. However, failure to include the essential information as exemplified in this model may be cause for either a delay in acceptance or the rejection of a bid or offer where the clause is Applicable. Further, the use of this model is not intended to waiver other requirements that may be applicable under FAR 52.219-9.
"SUBCONTRACT," means any agreement (other than one involving an employer-employee relationship) entered into by a Government prime contractor or subcontractor calling for supplies or services required for performance of the contract, contract modification, or subcontract.
