About this file
This is a Notice to Proceed Submittals checklist for SLAC National Accelerator Laboratory subcontractors. The document establishes mandatory requirements that subcontractors must submit before receiving authorization to proceed with work on a project.
The checklist requires subcontractors to submit Performance and Payment Bonds with penal sums equal to the Fixed-Fee-Price award for subcontracts valued at $35,000 and over. Approved insurance certificates must be provided for all subcontracts. For subcontracts of $25,000 and over involving design work by the subcontractor, Professional Errors and Omissions Insurance of at least $1,000,000 per claim is required. Subcontracts of $50,000 and over must include Builder's All Risk Insurance covering physical damage to work performed, with coverage equal to the full value of the work at the time of loss or damage, in a form acceptable to the University. Additional submittal requirements include the Statement and Acknowledgment Standard Form (SF1413) for all known sub-subcontractors, copies of the subcontractor's licenses and permits along with those of any known sub-subcontractors, Issue for Construction Set (IFC) Drawings and Specifications, and a project schedule in the format specified by the SLAC Project Manager. The document serves as a template with blank fields for Project name, Purchase Order number, Subcontractor name, SLAC Project Manager, and SLAC Construction Manager information.
