About this file
This is a Past Performance Questionnaire template designed to evaluate consultant performance on completed projects for source selection purposes during the proposal evaluation process.
The questionnaire is organized into two main sections. Section 1 requires contract identification information including the consultant/company name, project type classification, period of performance, initial and final project costs with explanations for any cost variances, scope of work description, and evaluator information including name, title, contact details, and months worked on the contract. Section 2 establishes a performance rating scale from 1 (Unacceptable) to 5 (Outstanding), with a Neutral option for items not observed or unable to rate. Evaluators assess consultant performance across two categories: Technical Performance, which covers ability to accomplish technical requirements, meet schedules, and provide quality data and analyses; and Management Approach, which evaluates overall project management effectiveness, personnel cooperation, problem resolution, workforce qualification and retention, subcontractor management, and quality control. The questionnaire permits up to two additional pages for supplemental information and is designated as source selection sensitive material for evaluation team use only.
