The Participant Training Evaluations provide the National Center staff information details regarding the status of the training course, course objectives and the satisfactory performance of the Contractor. This evaluation will be provided to each participant at the conclusion of each in-person training session and will be collected and sent to the Training Program Analyst, with all other training materials, after the end of each training session. For virtual trainings, the Training Program Analyst will distribute and collect evaluation forms electronically via email.

1. Format: A participant training evaluation for each training participant will be provided to the Contractor by the National Center in the box of materials shipped onsite for the training.

2. Schedule: All participant training evaluations shall be shipped to the Training Program Analyst, along with any remaining training materials, within 2 business days following the end of the training course. For example – the training course is started and concluded on a Wednesday, the return shipment of training materials and completed evaluations shall be shipped not later than Friday of the same week.
