Each category uses a standardized five-point rating scale: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, and Not Applicable (N/A). Respondents are encouraged to provide detailed explanatory narratives for each assessment. The questionnaire specifies that responses are deemed Source Selection Sensitive information and cannot be used for promotional or endorsement purposes by the contractor.

We are currently in the process of preparing our proposal for the Department of the Interior, National Park Service, Contracting Operations East, South Contracting Office, Solicitation Number:
140P5326R0022 for the Roadway Striping of GRSM. As part of our proposal, we have listed you as a reference for our performance for this type of construction service with your agency under the previous/current contract listed below. Your input on our performance is therefore requested.

Performance MEETS contractual requirements. The performance of the areas being assessed contains minor issues or concerns, for which corrective actions taken by the Contractor were effective.
2BMARGINAL
Performance MEETS SOME contractual requirements. The performance of the areas being assessed includes significant problems, issues, or concerns for which corrective actions taken by the Contractor were only somewhat effective.
3BUNSATISFACTORY
Performance DOES NOT MEET contractual requirements. The performance of the areas being assessed includes serious problems, issues, or concerns for which the Contractor’s corrective actions were ineffective.
