About this file

This is a Past Performance Questionnaire template for National Park Service RFP 140P5126R0008, which is an attachment to solicitation 140P5125R0014.

The questionnaire requests evaluations of a contractor's past performance on maintenance and repair work for unpaved roads at Big South Fork National River and Recreation Area (BISO) in Oneida, Tennessee. The primary deliverable requires providing a minimum of 16,100 tons of TDOT 903-specification crusher run limestone aggregate (¾-inch down to fines) or an NPS-approved equivalent, tailgate-spread uniformly across designated road segments at a three-inch average lift with plus or minus one-half inch tolerance. The contractor must use hauling equipment capable of accessing all roadways safely, including Duncan Hollow Road, Alfred Smith Road, Jack Ridge Road, and Big Island Road with minimum clearances of nine feet width and eleven feet height. The contractor is solely responsible for traffic control, flagging, and managing encounters with public vehicles on narrow roads without National Park Service support. The questionnaire must be submitted with the contractor's quote prior to the closing of RFP #140P5126R0008.

The evaluation template requests responses across five main assessment areas: overall Performance (ability to meet requirements), Timeliness (product delivery and problem resolution), Quality of Work, and Overall Contractor Performance rating. Evaluators use a standardized scale of Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable (N/A), with detailed definitions provided for each rating level. The template requires respondents to provide background information on the referenced contract, including contract number, value, period of performance, and description of services provided, along with the name and contact information of the person completing the questionnaire.

We are currently in the process of preparing our proposal for the National Park Service, Contracting Operations (ConOps) East, Solicitation Number: 140P5126R0008 that will provide Maintenance Repairs of Unpaved Roads - Big South Fork National River and Recreation Area (BISO) Onieda, Tennessee.

Provide Maintenance Repairs of Unpaved Roads by delivering a minimum of 16,100 tons of TDOT 903-specification crusher run limestone aggregate (¾-inch down to fines) or an NPS-approved equivalent.

Tailgate-spread aggregate in a uniform and continuous manner across the full width of travel along each designated road segment unless otherwise directed by the COR. Spot dumping, segregation, or uneven distribution is not permitted. Aggregate shall be tailgate-spread to achieve a uniform average lift of three

(3) inches, with an allowable tolerance of plus or minus one-half (0.5) inch. The contractor shall use hauling equipment capable of safely accessing all roadways. Most roads maintain approximate clearances of ten (10) feet width and twelve (12) feet height; however, Duncan Hollow Road, Alfred Smith Road, Jack Ridge Road, and Big Island Road contain tighter segments with minimum clearances of nine (9) feet width and eleven (11) feet height. The contractor is solely responsible for selecting appropriate equipment and ensuring safe passage without resource damage. The contractor must provide all traffic control necessary to safely deliver materials, including managing encounters with public vehicles on narrow roads. The National Park Service will not supply flaggers or escorts for Big South Fork National River and Recreation Area (BISO) Onieda, Tennessee.

As part of our proposal, we have listed you as a reference on our performance for this type of service with your agency under the previous/current contract listed below. Your input on our performance is therefore requested.
