About this file
This is a Past Performance Questionnaire used by the National Park Service to evaluate contractor performance for a service contract.
The questionnaire is part of RFQ 140P5126Q0017, issued under Solicitation 140P5124Q0030, for initial commissioning, inspection, preventive maintenance, and emergency services on Heating, Ventilation, and Air-Conditioning (HVAC) equipment at Martin Luther King, Jr. National Historic Park in Atlanta, Georgia. Prospective offerors reference past contract performance with the requesting agency, and the questionnaire is distributed to previous clients for evaluation. The form requests basic offeror information (name, phone, fax, email) and contract details (contract number, value, period of performance, description of items/services provided, and point of contact information).
The evaluation section contains five questions assessing contractor performance across multiple dimensions: overall ability to meet requirements, timeliness of product delivery, ability to resolve timeliness issues, quality of work, and overall contractor performance considering requirements, schedule, and price. Each question uses a six-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) with standardized definitions provided. Respondents are instructed to provide detailed narrative comments explaining their ratings. The completed questionnaire must be emailed to the Contracting Officer at edward_dingle@nps.gov.
