The questionnaire requests evaluation of contractor performance on a previous or current contract with the referenced agency. Respondents must provide contract information including contract number, value, period of performance, and description of items provided, along with the name and contact information of the person completing the questionnaire. The evaluation section uses a five-tier rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, and Not Applicable) to assess contractor performance across five dimensions: (1) overall ability to meet requirements, (2) timeliness of product delivery and ability to resolve timeliness problems, (3) quality of work and task requirements, and (4) overall contractor performance regarding requirements, schedule, and price. Respondents are instructed to provide detailed explanatory narratives for each assessment. The questionnaire explicitly states that responses constitute source selection sensitive information and contractors are prohibited from using the information for promotional or endorsement purposes.
