About this file

This is a Past Performance Questionnaire (PPQ) form used for solicitation W519TC-26-R-0007, a Demil and Disposal of Conventional Ammunition Demilitarization contract. The PPQ is a template designed to collect past performance information from references provided by offerors competing for this Army Contracting Command – Rock Island (ACC-RI) acquisition. Offerors must complete Sections IIA and IIB for each past performance reference and distribute copies to the identified respondents in a timely manner. The PPQ respondents—typically government contracting officers, contract technical representatives, or similar officials—complete the remaining sections and submit their evaluations directly to Liz Epperly, Contract Specialist, at Elizabeth.J.Epperly.civ@army.mil with a subject line clearly indicating the offeror name and solicitation number. Submissions must originate from the respondent's government or corporate email address.

The questionnaire uses a five-point rating scale (Excellent, Good, Satisfactory, Marginal, and Unsatisfactory) to assess contractor performance across five key evaluation areas: meeting contract quality and delivery requirements including specifications and workmanship; ability to deliver according to schedule; ability to identify and correct problems and prevent potential issues timely; satisfaction level of work effort performed; and compliance with OSHA and EPA regulations. Sections II-A and II-B collect respondent identification information and contract identification details, including prime contractor name, CAGE code, contract number, order number, product description, delivery quantities, monthly production rates, award date, contract end date, final dollar value, and place of performance. Section III-C gathers general contract information regarding the respondent's role, contract termination history, production quantity requirements, contract dollar changes, and issuance of Performance Quality Deficiency Reports (PQDRs) or Corrective Action Requests (CARs). Respondents provide narrative comments supporting each rating to substantiate their assessments based solely on contractor-liable performance.
