About this file
This is a General References form used by offerors to provide the Government with information about their financial, insurance, and operational support resources during the federal contracting proposal process.
The form requires offerors to disclose information about four primary categories of references: (1) Surety provider, including the surety's name, address, agent name, and telephone number; (2) Corporate banking relationships, with space for two separate banks and details such as bank name, address, agent contact information, telephone number, and types and numbers of accounts maintained; (3) Insurance company information, including the insurer's name, address, agent name, telephone number, and types of policies carried; and (4) Subcontractors and material suppliers, with space for three subcontractor entries and three material supplier entries, each requiring name, address, contact person, telephone number, and description of specialized work or material types. The form emphasizes that offerors must disclose any instances where relationships with references may be considered less than fully satisfactory, providing detailed explanations of remedial corrective actions taken or planned. Failure to make candid disclosures regarding relationship deficiencies may result in an unfavorable past performance assessment. All telephone numbers must include area codes, and explanations should be provided on separate sheets as necessary.

General References READ CAREFULLY: The purpose of this form is for the offeror to provide the Government with general references in the spaces provided as described in the Instructions, Conditions, and Notices to Offerors (Section L). Include Area Codes with all telephone numbers. Fill in the information by typing or printing legibly.
It is especially important that the offeror disclose instances in which their relationships with the references may be considered less than fully satisfactory. The offeror should tell their side of the story and describe remedial corrective action that has been taken or will be taken to correct the deficiency. Failure to do so may result in a determination that an offeror has been less than candid with the Government, which could result in an unfavorable assessment of the offeror’s past performance record. Provide explanations on separate sheets as necessary.
