About this file
This is a compliance attestation form for FAR clause 52.222-38 regarding veterans' employment reporting requirements under RFP No. 140P8626R0003 for the Conlon Creek Culvert Removal & Restoration project at Golden Gate National Recreation Area. The form requires offerors to confirm their compliance status with the Department of Labor's VETS-4212 reporting requirement, which mandates that companies awarded federal contracts valued at or above $150,000 in the past year must file the most recent VETS-4212 Report online. Offerors must select one of two checkbox options: either confirming they were awarded such a contract and have filed their most recent report, or confirming they were not awarded a contract priced at $150,000 or above in the past year and therefore have no filing requirement.
The form requires certification by an authorized company representative through signature, date, printed name, company name, and Unique Entity ID. This is a mandatory compliance document that must accompany the offer submission for the Golden Gate National Recreation Area culvert removal and restoration project administered by the Department of the Interior National Park Service Pacific West Region. The contracting officer for questions is Quinn Rankin at Quinn_Rankin@ios.doi.gov.
