About this file

This is a Quality Assurance Surveillance Plan (QASP) for Range 27 construction work at OPM-SANG, outlining performance requirements, standards, and surveillance methods for monitoring contractor performance. The plan covers seven key performance objectives: pre-construction submittals, road washout remediation, storm water prevention, transformer protection, range operations, security/access compliance, and as-built drawings/warranties.

The QASP establishes specific performance standards and Acceptable Quality Levels (AQLs) for each objective, including 100% compliance requirements for pre-construction submittals, >95% compaction for road reconstruction, ±2% slope tolerances for drainage systems, and precise specifications for transformer protection including 8-10 inch concrete thickness and IR reflector installation. The document details surveillance methods including periodic inspections, 100% inspection for critical tasks, random unscheduled inspections, customer feedback, and document reviews. It requires the contractor to conduct weekly progress meetings and mandates documentation procedures including Non-Conformance Reports (NCRs), with 1-3 working days for corrective action responses. The COR must maintain comprehensive QASP files and compile a final quality assurance report for contract close-out.

Quality Assurance Surveillance Plan Range 27 OPM-SANG Engineering and Construction

PERFORMANCE REQUIREMENTS AND STANDARDS

The primary performance objectives of this contract are outlined in the SOW. Below is a summary of key performance areas, derived from the SOW, along with their corresponding performance standards and acceptable quality levels (AQL).

Performance Objective Reference

(SOW

Section)

Performance Standard Acceptable Quality Level (AQL)

1. Pre-Construction Submittals (Site Grading/Road Reconstruction Plan, Storm Water Mitigation Plan, Transformer Protection Plan, QCP, Communication Management Plan, Submittal Plans, Inspection Plans, Mobilization Plan)

SOW §2.1

All submittals accurately address SOW requirements and are approved prior to commencement of construction.

100% compliance. No work on site may begin until COR has approved all required submittals.

2. Road Washout Remediation (Roads 1–4) SOW §2.2

Roads reconstructed in accordance with UFC 3- 250-0 and UFC 3-250-03, and materials/compaction meeting AASHTO and ASTM standards.

Surface and base course meet compaction >95%; no ruts, depressions, or irregularities > ±1 inch in 10 feet.

3. Storm Water Prevention (Ditch Remediation) SOW §2.3

Ditch cleared of debris and regraded for negative slope.

(EM 1110-2-1601,

AASHTO M288).

Ditch geometry and protective systems installed within tolerances (<±2% slope deviation).

4. Transformer Protection (Construction of Overhead Protection, IR Reflectors, Drainage, Erosion Repair)

SOW §2.4

Concrete overhead protection meets thickness, rebar, and drainage specs

(UFC 3-550-01). IR

reflectors installed for aerial recognition. Protection labeled with bright colors.

100% compliance with minimum thickness (8–10 in.), rebar placement, slope for runoff, and adequate labeling/IR reflectors.

Erosion at transformer base repaired and damp proofing applied.

Performance Standard Acceptable Quality Level (AQL)

5. Range Operations (Targeting Systems, Generator/Electrical System)

SOW §2.5

Systems tested, verified operational per manufacturer’s requirements. Any failures:

remediation plan submitted and approved prior to fix.

100% operational or a COR-approved remediation plan within 7 days of discovery of any failure.

6. Miscellaneous Security/Access & FPCON Compliance

SOW §2.6

Contractor and sub-contractors comply with security and access requirements, background checks, and local facility protocols.

100% compliance. No security violations, timely submission of access requests, and adherence to FPCON measures.

7. As-Built Drawings & Warranties

SOW

§§3.1, 3.2

Final as-built drawings reflect all changes and are submitted within 14 days after construction completion. Warranties cover all parts/installation per contract terms.

100% of as-built documentation submitted on time;

clear, correct, and complete. Warranties provided for all relevant components.

METHODS OF SURVEILLANCE

The Government will use the following surveillance methods to monitor and evaluate the Contractor’s performance. The COR/QAP may utilize one or more of these methods, as appropriate:

1. Periodic Inspections (Planned Sampling):

o Scheduled site visits by the COR or Government Inspector.

o Review of completed work for conformance with the SOW and approved submittals.

o Inspection of materials, measurements (e.g., compaction, thickness, slopes), and workmanship.

2. 100% Inspection (Critical Tasks):

o Used for tasks that are critical to project success or safety, such as verifying compliance with structural requirements for transformer protection or ensuring the correct slope for storm water drainage.

3. Random Inspections (Unscheduled):

o Conducted without notice to observe day-to-day performance and identify any potential non-conformities or deficiencies.

4. Customer/End-User Feedback:

o Feedback from OPM-SANG or other stakeholders regarding the functionality of roads, drainage channels, and range systems.

5. Document Reviews:

o Review of the Contractor’s quality control records, inspection forms, third-party testing reports, meeting minutes, and progress reports.

o Verification of compliance with submittal requirements (e.g., QCP, safety plan, mobilization plan).

PERFORMANCE MEASUREMENT AND ACCEPTANCE

CRITERIA

1. Evaluation Procedures:

o The COR or QAP will evaluate Contractor performance based on the surveillance methods described above.

o Each performance objective listed in Section 3.0 will be assessed for compliance with the performance standards and AQL.

2. Non-Conformance Documentation:

o Any non-conformance will be documented by the COR using a Non- Conformance Report (NCR).

o The Contractor must respond with a corrective action plan within a timeframe specified by the COR (typically 1–3 working days).

o Corrective actions must be approved by the COR prior to implementation.

3. Acceptance and Sign-Off:

o The COR will provide written acceptance if performance meets the standards.

o If rework is required, final acceptance is contingent upon correction of all deficiencies to Government satisfaction.

DOCUMENTATION AND REPORTING

1. QASP File:

o The COR will maintain a QASP file with all surveillance checklists, inspection reports, NCRs, and correspondence related to the contract.

2. Progress Meetings and Reports:

o The Contractor shall conduct regular progress meetings (at least weekly) in accordance with SOW §2.1.5 (Communication Management Plan).

o The Contractor shall submit progress reports (frequency established in the contract) summarizing completed activities, upcoming tasks, potential risks, and any quality issues or NCRs.

3. Final Quality Assurance Report:

o Upon project completion, the COR will compile a final quality assurance report indicating whether all contract requirements were met.

o This report will be used by the KO for final acceptance and contract close-out.
