About this file

This is an Invitation for Bid (IFB) offer form for the State of Illinois, submitted by a bidder in response to solicitation B-52287 for Non-VOIP Telephone System Maintenance. The Illinois Department of Innovation and Technology, in agreement with the Chief Procurement Officer for General Services (CPO-GS), seeks to establish an agency-specific master contract for non-VOIP telephone system maintenance services. The document serves as the bidder's formal offer to the State of Illinois and requires completion of all mandatory sections as outlined in the solicitation document section A.1, with failure to meet requirements resulting in disqualification.

The offer form addresses several key procurement elements, including provisions for bidders to claim Illinois Procurement Code preferences, such as the Domestic Product Preference, by marking applicable preferences and listing qualifying items with supporting explanations. Bidders taking exception to the Information and Technology Standard Terms and Conditions must submit a redline version with their offer, which the State may accept or reject at its discretion. The form requires the authorized representative of the bidding entity to sign and date the submission, certifying the bidder's commitment to full compliance with all solicitation requirements and terms.
