About this file

This is a Past Performance Information Sheet template for offerors responding to the Scott AFB Static Display Aircraft Restoration RFQ (FA440726QSK01). Offerors must submit three (3) past performance references that are recent (performed within five years of RFQ issuance) and relevant to airpark restoration work, specifically including restoration activities on military vehicles and equipment involving pressure cleaning and refinishing with durable exterior paint.

For each reference, offerors must complete a form documenting general project information, customer/agency details including a willing point of contact, contract value and period of performance, a detailed narrative describing work performed and its relevance to the RFQ's Statement of Work (including scope of restoration activities, military specificity, washing, repair, specialized painting/coating, and bird mitigation; magnitude such as similar number and size of aircraft; and complexity involving environmental, safety, and logistical constraints unique to military installations), and evidence of performance quality through available CPARS records, letters of recommendation, or other customer assessments. By submitting this information, offerors grant the U.S. Government permission to contact listed customer points of contact to verify performance claims.

Similar scope would also include restoration work on military vehicles and equipment that require the use of pressure cleaning and refinishing with durable exterior paint.

If a reference is for a project performed by a subcontractor or teaming partner, clearly identify the company that performed the work and describe their specific role in the project.
