About this file

The document provided is a Third Party Information System Access Request (MSC 0785) form from the Oregon Department of Human Services (DHS) and Oregon Health Authority (OHA). This form is used by DHS or OHA programs to request access for a third-party entity, such as an organization or individual, to data within a DHS or OHA information system or network. The form collects details about the third-party entity, the governing contract, the reason and method of access, as well as the program sponsor and requestor. The form covers access to various DHS and OHA systems, including Child Welfare, MMIS, and others, and identifies the type of information that may be accessed, such as personally identifiable information, financial information, and criminal justice information.

The document does not contain details about a specific state and local contract opportunity. However, it is related to the management of third-party access to DHS and OHA information systems, which could be relevant to vendors providing services to these agencies under state and local contracts.

Definition:

The term “SSA data/information” also includes information provided to the electronic information exchange partners (EIEPs) by a source other than SSA, but which the EIEP attests to that SSA verified it, or the EIEP couples the information with data from SSA as to certify the accuracy of the information. The following are examples of SSA information:

• SSA’s response to a request from an EIEP for information from SSA (e.g., date of death)

• SSA’s response to a query from an EIEP for verification of an SSN

• Display by the EIEP of SSA’s response to a query for verification of an SSN and the associated SSN provided by SSA

• Display by the EIEP of SSA’s response to a query for verification of an SSN and the associated SSN provided to the EIEP by a source other than SSA

• Electronic records that contain only SSA’s response to a query for verification of an SSN and the associated SSN whether provided to the EIEP by SSA or a source other than SSA.: 0
