About this file
This document is Addendum #6 to the Oregon Educators Benefit Board's (OEBB) Employee Assistance Program (EAP) Request for Proposals (RFP) under OregonBuys Bid No. S-44300-00008777. The RFP seeks proposals from qualified vendors to provide EAP services to employees of OEBB entities participating in OEBB's EAP Program. The RFP schedule has been amended, with the proposal due date, Attachment D and Disclosure Exemptions submission date, and instructions for claiming disclosure exemptions changed. The RFP also clarifies the minimum requirements for proposals, including that EAP services must be available to employees not enrolled in a medical plan, their dependents, and household members who are not dependents, and must be available at both physical locations and virtually.
The RFP requires proposers to submit a complete and responsive proposal, including required attachments and certifications. Proposers must agree to a number of representations and warranties, and some responses may be limited in length. OEBB requires proposals to comply with all RFP specifications, and any missing information could disqualify a proposal.
