About this file
This document consists of two affidavits required by the State of Florida for the Elevator Maintenance and Service Bid #27-0030 issued by the School Board of Sarasota County, Florida. The first affidavit, Attachment F, requires vendors to certify under penalty of perjury that their company does not use coercion for labor or services as defined in Florida Statutes Section 787.06(13). Vendors must affirm they do not engage in practices including physical force, unlawful restraint or confinement, exploitative debt arrangements, document confiscation, financial threats, fraud, or provision of controlled substances. The second affidavit, Attachment G, requires vendors to certify that their company is not owned or controlled by a foreign country of concern, specifically the People's Republic of China, Russian Federation, Islamic Republic of Iran, Democratic People's Republic of Korea, Republic of Cuba, Venezuelan regime of Nicolas Maduro, or Syrian Arab Republic, as prohibited under Florida Statutes Section 287.138. Both affidavits must be signed by an authorized officer or representative of the responding vendor and submitted either prior to award or with the bid submission.
These affidavits are mandatory compliance documents that all nongovernmental entities bidding on this contract must complete and submit. The affidavits require the vendor to provide standard information including company name, FEIN, authorized representative details, address, phone number, and email address. Signatures must be executed under penalty of perjury, establishing legal accountability for the certifications made. The document was approved by the School Board's General Counsel on April 24, 2024, and serves as both a pre-award certification option and an alternative submission method allowing vendors to submit these documents concurrently with their bid proposals.
