About this file

This is a Past Performance Questionnaire (PPQ) template designed for the Department of Veterans Affairs Veterans Health Administration (VHA) to evaluate contractor performance on past projects relevant to the solicitation Z1DZ--544-21-115 B7 HVAC & Electrical Upgrade Construction (RFQ 36C24726Q0685).

The form is divided into two sections: Section 1 requires the offeror to provide contractor information, contract details, and project descriptions for past performance examples, including contract numbers, award dates, contract acceptance dates, original and final contract prices, and project scope. Section 2 is completed by the client/evaluator to assess the contractor's past performance across seven criteria: quality of design and construction standards, schedule and timeliness of performance, communication and cooperation, management and personnel continuity, cost and financial management, subcontract management, and overall recommendation for future work. Evaluators select from adjectival ratings (Outstanding, Above Average, Satisfactory, Unsatisfactory, or Not Applicable) with supporting comments for each category. The completed PPQ is to be incorporated into the offeror's SF 330 submission and returned to the contractor, not submitted directly to VHA-NCO-7, by the RFQ submission due date specified in the solicitation.
