About this file
This is a certification form attachment (Attachment N) related to the Agriculture and Lawn Equipment Invitation to Bid (ITB-V2) issued by the Florida Department of Management Services, Division of State Purchasing. The solicitation seeks to establish a state term contract for agriculture and lawn equipment with the potential for multiple statewide awards at the Department's discretion. The Department reserves the right to award to one or multiple bidders, regionally or statewide, or to make no award based on the state's best interest. Responses must be submitted through the MyFloridaMarketPlace Sourcing Event via the SAP Business Network, with vendors responsible for monitoring the Vendor Information Portal (VIP) throughout the procurement process. The procurement officer is Chris Molina, Purchasing Analyst, and all email communications regarding this solicitation should include the solicitation number in the subject line.
Attachment N specifically requires that a member of the company's senior management—defined as chief executive officers, assistant CEOs, chief financial officers, chief personnel officers, or employees performing similar functions—certify that the commodities being offered have not been produced in whole or in part by forced labor, in accordance with Florida Statute section 287.1346(4)(b). The certification must be completed at the time of bid submission and before contract execution or renewal. The form requires the entity name, senior management's printed name and title, signature, and date. Protests of agency decisions must be filed with the Department of Management Services Office of the General Counsel within the timeframe prescribed in Florida Statute section 120.57(3), or such protests are waived.
